Each submission of a professionalism concern or mistreatment report will be reviewed by the CHM Student Resolution Advocate only. No response can be sent back to the sender unless that information is included on the form as submissions are anonymous.

Confidential, non-anonymous reports with sufficient detail are most useful in support of follow up however you may submit an anonymous report.
